The clinical pharmacy specialist is primarily responsible for providing pharmaceutical services to adult patients of all ages in accordance with approved policies and procedures and accepted pharmacy practice.  Responsibilities include interpreting medication orders written by providers; counseling patients on proper medication administration and storage; evaluating patients medication therapy, making recommendations to providers, performing medication reconciliation according to guidelines and policy, provide education to staff/patients, provide anticoagulation dosing/monitoring, and participate as voting and non-voting member on committees as requested/required, attend Interdisciplinary (IDT) rounds daily to provide direct patient care.  Incumbent is also a back-up clinical pharmacist for supporting patients in Home Based Primary Care (HBPC) and investigative drug studies.  Clinical Pharmacist Specialist may also manage medication programs as requested/required.

The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients.  A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.  This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.  Pharmacists assigned to this position must demonstrate the following KSAs:

  a.   Ability to communicate orally and in writing to persuade and influence clinical and management decisions.

    b.   Expert understanding of regulatory and quality standards for their program area.

  c.   Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

  d.   Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

  e.   Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

BASIC REQUIREMENTS: 

·  U.S. citizenship. 

·  Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy 9BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.  Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education website at Http://www.acpe-accredit.org/ 

·  Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S. or the District of Columbia;

·  Proficiency in spoken and written English and 

·  Must meet the physical requirements of the position. 

Specialized Experience: 

In addition to meeting all basic requirements, applicants must have one year of specialized experience as a Clinical Pharmacy Specialist at the GS-12 level in the Federal Service or equivalent; or board certification recognized by the American Pharmaceutical Associations Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent) or completion of one year post-Pharm.D. ASHP fellowship in a clinical or specialized program.  Specialized experience includes: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

PHYSICAL REQUIREMENTS: Requires sitting for long periods of time, bending and some light lifting (up to 15 lbs.) will be required.  Mental stability and able to work under stressful situations.
